Patient Forms and Resources

The staff at Precision Surgery Center of Napa Valley looks forward to meeting you and discussing what you can expect throughout your surgical experience. We strive to keep our surgical procedures on time as scheduled. Prior to your scheduled surgery, we encourage you to fill out the patient forms that are located on this website to accelerate the admission process. Please download, print, and complete the forms and bring them with you on the day of your surgery. If you are unable to download our patient forms, then it is recommended that you arrive for your procedure early so that you have ample time to fill out the appropriate forms. 

Initial Consultation

You should plan to arrive at our Napa, CA, surgical center at the arrival time designated by our staff. Please bring a list of medications you are currently taking, including any eye drops, on the day of surgery.

Important: All patients under 18 must be accompanied by a parent or guardian. 

Depending on the type of procedure you are receiving, your eyes may be dilated, and you may consider arranging a ride home. 

Referrals

If your doctor has referred you, he or she will promptly receive a letter outlining our findings and the course of treatment. We encourage you to return to your referring doctor after our specialized care is complete.
